ZorgDomein Integrator supports the full process of the nursing transfer (verpleegkundige overdracht). This process consists of three different steps which are all facilitated by our interface:
- Sending a placement request
- Answering a placement request
- Sending a transfer dossier
Sending a placement request
The first step in the transfer process is sending a placement request. This is done by a transfer nurse to ask whether a specific healthcare supplier is able to admit a specific patient at a specific date. To send a placement request, the transfer nurse logs in to ZorgDomein directly from his/her own information system using SSO. After logging in, he/she will select a transfer product that is published by a specific healthcare supplier (the transfer product contains an indication of the availabilty for new patients). This healthcare product determines the choice for the healthcare supplier. Once the healthcare supplier has been selected, the transfer nurse will compose and send a placement request form to the healthcare supplier. This document contains the request for placement and provides some relevant medical data about the condition of the patient. This data is automatically added to the request form as much as possible based on the medical patient data from the information system of the transfer nurse. The transfer nurse may also add attachments to the request form. These attachments should be fetched from the information system of the transfer nurse. After submitting the request form, ZorgDomein will deliver the request form at the information system of the selected healthcare supplier. A copy of the request form will be sent back to the information system of the transfer nurse for archiving purposes.
Answering a placement request
When the healthcare supplier has received the placement request, an authorized practitioner can respond to the request by sending an answer to the transfer nurse which indicates whether the placement request is accepted or not. The practitioner starts this report from his/her information system by logging in to ZorgDomein using SSO. After logging in, he/she will be directed to the answer form that can be used to compose and send the answer. After submitting the answer form, ZorgDomein will deliver the corresponding answer document to the information system of the transfer nurse. A copy of the answer document will be sent back to the information system of the healthcare supplier.
Note that the answer to the placement request is implemented as a new document in the same transaction as the placement request. This implies that the answer document will have the same ZD-number as the placement request form.
Sending the transfer dossier
Once the transfer nurse has received a confirmative answer to the placement request, the actual transfer dossier can be sent to the selected healthcare supplier. To send a transfer dossier, the sender of the dossier logs in to ZorgDomein directly from his/her own information system using SSO. After logging in, he/she will be directed to the transfer flow. In this flow the sender can select the recipient for the transfer dossier (the recipient should match the healthcare supplier that has accepted the placement request). After the recipient has been selected, the sender is directed to the transfer form that can be used to compose the transfer document accompanying the transfer dossier. The content of this transfer document is automatically composed as much as possible based on the medical patient data from the information system of the sender. The sender will most probably add the transfer dossier as one or more attachments to the transfer form. These attachments should be fetched from the information system of the sender. After submitting the transfer form, ZorgDomein will deliver the transfer document including the transfer dossier and additional attachments to the information system of the selected recipient. A copy of the transfer document will be sent back to the information system of the sender.
Note that the sumbission of the transfer dossier is implemented as a new transaction. This implies that the ZD-number of the transfer document differs from the ZD-number of placement request form and answer document.
